We acknowledge that we have read the Clarification/Information Text provided to us. Within the framework of the conditions set forth in the “Clarification Text,” we consent to the processing of our company’s personal data by the Clinic, acting as the data controller. This data is clearly related to an identified or identifiable natural person, and is processed partially or completely by automatic means or non-automatically as part of a data recording system. The data includes: Identity Information of our company and its authorized representatives (Name, surname, SSI number, Turkish ID number, tax number & registered tax office), Contact Information of our company and its authorized representatives (Address, Phone number, E-mail Address), financial, accounting, and payment-related company data, and other personal data of our company necessary for the execution of the service relationship between the parties;

-For keeping video records inside and around the Clinic,
-For the purpose of carrying out activities within the scope of the service relationship,
-For the purpose of updating and verifying the accuracy of the transmitted information,
-For ensuring the proper fulfillment of obligations within the scope of the contractual relationship,
-For the Execution of Activities in Compliance with Legislation
-For the Execution of Business Continuity Activities
-For the Execution of Service Procurement Processes
-For the Execution of the Supply Chain
-For the Execution of Communication Activities
-For the Execution of Service Loyalty Processes
- For the planning and execution of corporate sustainability, corporate governance, strategic planning, and information security processes
-For the purposes of checking powers of attorney and signature circulars,
to be processed for the period stipulated in the relevant legislation or required for the purpose for which they are processed, through operations such as obtaining, recording, storing, preserving, modifying, reorganizing, disclosing, transferring, taking over, making available, classifying, or preventing their use, and;
For the purpose of fulfilling the Clinic's legal obligations;
-To authorized institutions and organizations, lawyers, financial advisors;
-To the financial, accounting, and purchasing/marketing departments of the parties, their lawyers, financial advisors, and banks for the purpose of carrying out finance, accounting, and payment transactions, contract processes, goods/service purchase/sales processes, and product/service marketing processes;
-We agree to the transfer to contracted IT consulting firms, customers, suppliers, and servers located domestically and abroad for the purpose of carrying out business and communication activities and the service relationship, the company's physical security and information security processes, and storage and archiving activities.
